What VO2 max measures, and why it matters so much
VO2 max is the maximum amount of oxygen your body can use during intense exercise. It is measured directly. You wear a mask that analyzes the air you breathe out while the workload increases, so the result is a measurement rather than an estimate.
It is worth caring about because it is among the strongest predictors of long-term mortality we have. Its predictive power is comparable to or stronger than that of smoking status, blood pressure and cholesterol. And unlike most predictors, it responds to training at any age and from any starting point.
The counterintuitive part: the largest gains in risk are at the bottom of the range. Moving from very low fitness to merely below average is a bigger reduction in risk than moving from good to excellent. That means the test matters most for people who assume it is only for athletes.
Resting metabolic rate, and why the calorie number you have been using is probably wrong
Resting metabolic rate is how much energy your body uses doing nothing. It is the largest share of your daily energy use, and the online calculators everyone relies on are population averages that can be substantially wrong for an individual.
That gap explains a lot of frustration. If your actual rate is well below the estimate you have been eating to, you have been in a surplus while believing you were in a deficit. The estimate was wrong, and your effort was never the problem.
Measuring it replaces a guess with a number. Sometimes the measurement also uncovers something worth treating. A rate well below prediction can point to a thyroid problem, a medication effect, or the metabolic adaptation that follows years of restrictive dieting.
What we do with the result
Your physician goes through the results with you: where you sit for your age and sex, what the results mean for your risk, and what would improve them. Training prescriptions that come out of a real measurement are considerably more specific than general advice to exercise more.
The results also inform the rest of your care. A low VO2 max with normal bloodwork is worth investigating. A resting rate well below prediction is worth explaining. And repeating the test in a year shows you whether what you changed actually worked.

What people ask
Do I need to be an athlete for this to be useful?
No, arguably the reverse. VO2 max is one of the stronger predictors of long-term health at every level of fitness. Knowing your starting point matters most when that starting point is low.
How often should it be repeated?
Usually annually, or after a meaningful change in training or treatment, so you can see whether what you are doing is working.
What does the test involve?
You exercise on a bike or treadmill with a mask that measures the air you breathe out, while the workload increases gradually to your maximum. It takes about fifteen minutes of actual effort, and it is hard for the last few minutes by design.
Is it safe if I am not fit?
For most people yes, and it is supervised throughout. Where there is cardiac risk we assess that first and sometimes recommend a submaximal version instead. A submaximal test still gives a useful estimate without pushing you to your true maximum.
What is a good VO2 max?
It depends entirely on age and sex. What matters is where you sit within the range for your age and sex. The more useful question is not whether it is good but whether it is improving.
