What you should know
Low testosterone has causes, and some matter more than the number
Obesity, sleep apnea, poor sleep, significant stress, alcohol, opioids and several common medications all suppress testosterone. So do problems with the testes or the pituitary. These are less common, but they are considerably more important to identify.
Treating the number without asking why it is low leaves the underlying cause unaddressed. In several of those cases, treating the cause raises testosterone on its own, without lifelong therapy.
We ask first not to make it difficult, but because starting therapy closes off the question permanently, and it is worth asking once.
What monitoring involves, and why it is not optional
Testosterone therapy has to be monitored: levels to confirm you are in range, a blood count because it can thicken the blood, and prostate monitoring where appropriate. We also ask whether you feel better, because feeling better is the point of treatment.
It also has consequences worth understanding before starting, including effects on fertility that are not always reversible. That belongs in the conversation at the beginning rather than as a surprise later.
Where we are not the answer
We are not a testosterone clinic and we will not start therapy on request without an evaluation. If your levels are normal we will tell you, and we will look for the real reason you feel the way you do.

What people ask
Will you prescribe testosterone if my levels are borderline?
Sometimes, and it depends on far more than the number: symptoms, cause, your goals, and the risks in your particular case. We make that decision with you, and the number is only one part of it.
Will testosterone therapy affect my fertility?
It can, significantly, and the effect is not reliably reversible. If you may want children this changes the conversation substantially and there are alternative approaches worth discussing first.
Do I need therapy forever once I start?
Frequently yes, because taking it suppresses your own production. Some causes of a low level are treatable on their own, without committing to lifelong therapy.
Can you handle erectile dysfunction?
Yes, as normal primary care. It is also worth knowing that it is often an early cardiovascular sign, so it deserves a proper look rather than just a prescription.
