Who we serve
Peace of mind that your parent's care is handled
You stop being the one holding your parent's care together. With your parent's consent, we bring you into the plan properly. You no longer have to piece together what happened from phone calls, results you chased yourself, and flights booked at short notice.
Most of the people who call us about a parent live somewhere else.

Sound familiar?
What usually brings someone to us
You find out afterward
The appointment happened, something was decided, and you hear a partial version of it days later, if at all.
Coordinating the specialists falls to you
A cardiologist, a nephrologist and a primary doctor each have a plan, and the notes sit in three different offices. You are the one carrying information between them.
You cannot get anyone on the phone
You are not the patient, so the portal will not let you in and the office cannot tell you anything. Yet you are the one making the decisions.
The call from the hospital overnight
Something happened overnight, you are in another time zone, and you need to reach someone who knows your parent's history.
What we do about it
What changes when we are your practice
We write down who to call, and about what
With your parent's consent, we record exactly who should be contacted, by which method, and for which kinds of news. So when something happens, nobody is guessing.
You can be on the call
Join the visit by phone or video. You hear the same explanation your parent hears, at the same time, and you can ask your own questions.
One physician has your whole chart
Making sure the cardiologist's plan and the primary plan agree is our job. You are not the one relaying decisions between offices.
We visit them in the hospital
If your parent is admitted, their physician comes to see them and talks to the team treating them. Your physician is there in person and speaks to the hospital team directly.
The plan is in writing
You get the same document your parent gets: what was found, what is being done, and what happens next. Something you can act on from anywhere.
Where we are not the answer
We are not a home-care agency and we do not provide aides, nursing visits, or house calls. If your parent needs someone physically present day to day, we will point you toward a service that provides that.
Questions
What people ask
Can I be involved if my parent is competent and independent?
Yes, and it is your parent's call, not ours. They decide how much you hear and about what. Most people choose more involvement than the default once we ask them.
Do you do house calls for elderly patients?
No. A physician does not come to the house. We do virtual visits and visit patients in the hospital, and home blood draws come with an optional membership tier.
My parent is only in Naples for part of the year. Does this still work?
Yes, and it is common. We send your parent's records where they are needed, and we take calls from anywhere. If something happens while they are north, we will find them care there and speak to the physician treating them.
Do you handle dementia and memory concerns?
Yes. As the primary practice, we provide dementia and Alzheimer's care and advance care planning.

What is your medical care missing?
A short conversation with our member liaison about the care you have now: who follows up, who keeps your care coordinated, and where the gaps are. We will tell you if there are no gaps.
