Concierge care means you keep one physician, and that physician keeps a panel small enough to know your history. The practice also coordinates between your specialists so you do not have to. Membership is annual, it is not insurance, and it does not replace the insurance you have.

If you would rather ask than read, book a complimentary clinical gap assessment. The call takes fifteen to twenty minutes.

What changes with a concierge physician

Your prevention plan is built around your own history rather than a population average. One team coordinates your specialty care, and you keep the same physician long enough for the relationship to matter.

Customized prevention

A plan built from your own history, risk profile and goals rather than a population average. Advanced cardiac and lipid panels, and nutrition and lifestyle counseling from the physician who also knows your labs and your medication list. Metabolic and VO2 max testing come with an optional membership tier.

Care navigation

One team coordinating your care across specialties. We request your records from every physician you have seen and transfer them ourselves, we make the referrals, and we follow up on the results rather than waiting for you to ask.

Collaborative partnership

A physician with a panel small enough to know where you are and where you are going. You reach that physician by text, and same-day and next-day appointments are the normal case. If you are admitted to the hospital, your physician visits and speaks to the team treating you.

How it compares

What is different from a conventional primary care practice?

The difference is the panel size, and everything else follows from it. A physician responsible for a few hundred people can do things a physician responsible for two thousand cannot.

A conversation about the care you have now

You spend fifteen to twenty minutes on the phone with our member liaison for a complimentary clinical gap assessment. They ask what your care looks like today and answer your questions about how membership works. There is no obligation.

A meeting with a physician

If going further makes sense, you meet one of our physicians in person before you decide anything. This meeting is also complimentary.

You decide in your own time

You leave the meeting with a clear picture of what membership includes and what it costs, and you decide in your own time.

We handle the transfer

Once you join we request your records from every physician you have seen, transfer them ourselves, and read them before your first visit rather than at it.

Where we are not the answer

Concierge care is a better primary care relationship. It is not a second opinion service, and it does not replace specialty or emergency care. If what you need is a cardiologist, an oncologist or an emergency room, we will say so and help you get there.

Questions prospective members ask

What is concierge medicine?

Concierge medicine is primary care paid for by an annual membership rather than per visit. Because the membership funds a much smaller panel per physician, we can offer longer appointments, direct access to your own doctor and coordination between specialists. Membership is not insurance and does not replace the coverage you have.

Do I still need health insurance if I join?

Yes. Membership covers your primary care relationship with this practice. Your insurance continues to cover specialists, hospital care, imaging, prescriptions and everything else outside this office, and you should keep it.

What does membership cost?

Membership is an annual fee with no per-visit billing on top of it. Our member liaison will give you the current figure during your clinical gap assessment, and you will have it in writing before you are asked to decide anything.

How many patients does each physician take?

Each physician keeps a small fraction of a conventional panel. In a conventional practice, one physician may have two thousand patients. The whole point of this model is that the number is low enough for your physician to know your history without reading it back from a screen.

Who is a good fit for concierge care?

It suits people managing several conditions at once, and people who travel or spend part of the year in Naples. It also suits adult children coordinating a parent's care from out of state, executives whose health has become one more thing to manage, and people who are well and intend to stay that way. We accept patients from age 6, so it also works as one practice for a whole household.

Can I speak to a physician before joining?

Yes, and meeting a physician is complimentary. Your clinical gap assessment with our member liaison comes first. If the model looks like a fit, the next step is meeting a physician in person before you commit to anything.

How quickly can I be seen once I join?

Same-day and next-day appointments are the normal case rather than a favor, and members reach their physician directly by text between visits.
